How to Apply
Candidates who meet the eligibility criteria outlined above are invited to participate in the Walk-in Interview scheduled for the specified date, time, and location. They must bring a completed Application Form (Annexure-1), along with two recent passport-sized color photographs and the original certificates/documents listed below, accompanied by self-attested photocopies:
a) Proof of Date of Birth (Class X/Matriculation certificate from a recognized Board or Council/Birth certificate issued by the appropriate authority)
b) Class XII Mark Sheet
c) MBBS Mark Sheets for all years
d) MBBS Degree/Passing Certificate
e) MBBS Registration Certificate (from the Medical Council of India (MCI), National Medical Commission (NMC), or State Medical Council (SMC), or a valid practitioner license enrolled in the State or National Register)
f) Internship completion certificate (for GDMOs)
g) PG Diploma/PG Degree Mark Sheets
h) PG Diploma/PG Degree Passing Certificate
i) PG Diploma/PG Degree Registration Certificate (from MCI, NMC, or SMC, or a valid practitioner license enrolled in the State or National Register)
j) Valid practitioner license
k) Experience Certificate demonstrating relevant post-qualification experience from the employer (for retired employees, a service certificate from the parent organization is required)
l) Photo Identity Proof (such as Aadhaar Card, PAN Card, Driving License, Passport, or Voter ID)
m) Address proof (Voter ID, Driving License, Aadhaar Card, or other documents)
n) Category/Caste Certificate SC/ST/OBC (NCL)/EWS and PwBD Certificate (if applicable). Candidates belonging to the "CREAMY LAYER" of the OBC category are not eligible for OBC concessions and must indicate their category as "General." OBC (non-creamy layer)/EWS candidates must provide the necessary certificate in the prescribed format issued in the current financial year (2024-25). Caste certificates should be submitted accordingly.
